Non-DoD Participants & Spectators Affidavit for Access to Eglin AFB

Participants can drop off their affidavits to the East Gate Visitor Control
Center (VCC) between 6:00 a.m. to 2:00 p.m. or at the West Gate VCC from 6:00 a.m. to 10:00 p.m. or send it
in to the address below. Please note that if mailing in your Affidavidt via USPS, all forms must be postmarked no later than 16 May 2016, any forms postmarked later than date indicated will not be honored and participants will not be allowed access to Eglin Air Force Base.
